Request a Delivery of Your Portable Restroom Unit

By checking this, I agree to the TERMS & CONDITIONS provided in this form:

1. The customer agrees to pay rental unit fees and any added services no later than 10 days from the invoice date. If payment is received more than 20 days from billing term, there will be a $1.00 per day late charge per billing cycle.
2. There is no proration on a billing cycle.  If the unit goes into a new billing cycle the cost is the same if it is there one day or the entire time.  To avoid added fees please make sure to call or email your removal request before the end of your billing cycle.
3. Customer agrees to keep portable toilet facilities clear and accessible for the service truck at all times. If the facility is blocked, we will charge an additional fee for any added visit to service the facility.
4. The customer agrees not to sell, rent, lease, move or otherwise give possession of portable toilet facilities to anyone except to Port-O-Potty INC.
5. The terms of the contract will be the same at this location as well as all future jobs and / or location unless otherwise agreed upon by both companies.
6. Customer agrees to keep equipment protected and properly secured during term of their contract.  This also includes all weather conditions. Customer also agrees to pay for any damage or loss of any equipment.
7. Customer understands that nothing can be added to the unit except by a staff member.  This includes hasps, locks and / or hand sanitizer.
8. Customer acknowledges receipt of the property listed above and inspected it at the time of delivery.
9. Port-O-Potty, INC. may take possession of equipment at any time if payment has not been received within the rental agreement terms.
10. Customer is responsible for all cost of collections including attorney fees together with added fees due to Port-O-Potty, INC.
11. All payments by check or money order are to made payable to: Port-O-Potty, INC.  If payment is being made by credit card the customer agrees to pay for unit in full without fail unless otherwise agreed to in writing by our office staff.
